Please follow the rest of the steps below to complete your return authorization request.

1. Parts must be returned in their original packaging and packaged in a manner to prevent damage.
2. International shipments: Please provide to your shipping company / freight forwarder a copy of the "Declaration of Foreign Shipper" form for US made goods being returned to country of origin for warranty / repair (Optical filters #9801.00.90)
3. Clearly mark the RMA number on the outside of the shipping container and place a copy of the return authorization with the returning products.
4. Return all parts to: IDEX Health and Science | Semrock, 1180 John Street, Rochester, New York 14586 Attn: RMA#
5. Credit or replacements will be handled once the returned part(s) are received and inspected.
